In response to COVID-19, walk-in services are closed.

Birth and death certificates may continue to be ordered by mail. 

 


Who Can Order Certificates

IMPORTANT NOTE: Starting January 1, 2021, a new vital records law and rules going into effect that change the ordering requirements for birth and death certificates

  • For increased security of personal information, only individuals with specific relationships to the person on the record being requested can receive a certificate.
  • Identity and proof of relationship documentation will be required.
  • Certificate fee will increase to $25.00 per copy.

How to Obtain a Birth Certificate

1) Complete all fields on the Birth Certificate Order Form sign, and date 

2) Provide a copy of acceptable proof of identity

3) Provide a copy of your proof of eligibility document

4) Send or bring the completed order form, all documents, and nonrefundable payment to the address listed below.

1) Complete all fields on the Death Certificate Order Form sign, and date 

2) Provide a copy of acceptable proof of identity

3) Provide a copy of your proof of eligibility document

4) Send or bring the completed order form, all documents, and nonrefundable payment to the address listed below.

Birth and death certificates may also be ordered online through www.vitalcheck.com. VitalCheck can issue birth and death certificates for other states as well.

What form of payment is accepted? 

Walk-in: Cash, Check/money order, credit/debit card

Mail-in: Check/money order payable to NETCHD (DO NOT MAIL CASH)
