2) Provide a copy of acceptable proof of identity
3) Provide a copy of your proof of eligibility document
4) Send or bring the completed order form, all documents, and nonrefundable payment to the address listed below.

Birth and death certificates may also be ordered online through www.vitalchek.com. VitalChek can issue birth and death certificates for other states as well.
